A case of trichotillomania in a patient with narcolepsy

A 19-year-old well-nourished girl presented with a 4-year
history of hair loss. On past history, she has been treated
with narcolepsy for four years. She had a low self-esteem
and was distressed for her obesity. On her frontal and left
temporal area, several ill-defined patches of hair loss were
seen. And the hairs in that area, they are short, irregular,
broken and distorted. She denied the fact she pulled out hairs,
but in the morning, there were several hairs on her bedside.
Histopathologic exam shows an increase in catagen hairs, pigmentary
defects and casts, and trichomalacia. We herein report a case
of trichotillomania occurring on the patient with narcolepsy.
It is uncertain that the narcolepsy affects the development
of trichotillomania, but her depressed mood may affect the
development of narcolepsy as well as the development of trichotillomania.
