With regard to Congenital and Hereditary Hair Shaft Abnormalities our knowledge has now reached the stage where for many of them the exact molecular genetic fault has been defined. However if treatment is to become more specific in relation to this the clinician still has to make an accurate clinicopathological diagnosis. It is therefore apposite at this stage to reflect back on the evolution of hair structure as it has evolved through clinical dermatology and genetics, light and electron microscopy and biochemistry.
