PHENOMENON IN TRICHOLOGY

The Köbner phenomenon (KP) was first described in
patients with active psoriasis; since then the term of KP or “reactive
isomorphism” has been utilized by dermatologists in several cases where the
underlying disease “emerges” after trauma (traumatic injuries to the skin).
Objectives: In our presentation, the clinical evidence of KP in
some hair diseases will be described. We report on cases of Lichen Planus-
Pilaris, Pseudoarea of Brocq and Lupus Erythematosus of the scalp which were
exacerbated by spontaneous or iatrogenic trauma, and describe our experience of
cases with KP and its variants, such as “inverse-KP” and “Rembök-phenomenon” in
Alopecia Areata.
Conclusions: Considering the possible role of mast cells as
triggers of KP in psoriasis, their implication in the cases described will be
suggested on the basis of histological evidence.
The possible involvement of neuromediators like substance
P in the pathogenesis of KP through mast cell activation will also be
discussed, thus underlining the importance of “psycho-“ or “neurogenic” stress
as a potential trigger factor in various clinical forms of alopecia.
Finally, we believe that the identification of this
phenomenon may explain why some treatments are ineffective or even
exaccerbate the clinical picture when utilized in the active phase of the
disease -even in the same patients where this therapy had been previously
efficacious.
