S4 SEMI-INVASIVE METHODS: THE VALUE OF TRICHOGRAMS II

The value of trichogram is in my experience restricted to 2 main pathologies:
androgenetic alopecia and loose anagen hair syndrome. In androgenetic alopecia,
trichogram is a complimentary tool and can be substituted by pull test and
dermoscopy. If performed, trichogram should be done both in the androgen-dependent
affected areas of the scalp and in the occipital scalp, for comparison. The
hair from the affected scalp shows diameter variability and a decreased anagen/telogen
ratio. Loose anagen hair syndrome includes severe cases, that can be easily
diagnosed by pull test with extraction of a large number of loose anagen hair
-anagen hair devoid of root sheaths and with the typical "ragged-sock" cuticle,
and less severe cases, where trichogram is mandatory for diagnosis. A percentage
of more than 70% of loose anagen hair at the trichogram is diagnostic for the
disease. In these patients trichogram may also be utilized in serial visits,
to monitor the frequent spontaneous improvement of the disease with age.
