ALOPECIA AREATA IN A PATIENT WITH BASAL CELL CARCINOMA ARISING ON NEVUS SEBACEUS

A 32-year-old Korean man presented with a well-demarcated verrucous plaque
and a surrounding alopecic patch on parietal scalp. The yellowish plaque on
scalp was noted at birth, thus it has been known as nevus sebaceous clinically.
But, six years ago, firm brownish nodules started to appear on it and multiple
alopecic patches was developed including the one around it. He has been treated
with intralesional steroid injection at other hospital, under clinically diagnosis
of alopecia areata. His hairs show re-growth, but there was no effect on the
perinevoid alopecic patch. We perform a biopsy on brownish nodules and a histopathologic
examination revealed a basal cell carcinoma arising from nevus sebaceus. After
complete excision of nevus sebaceus with associated basal cell carcinoma, the
perinevoid alopecic patch disappeared spontaneously. It is presumed that autoantibodies
to basal cell carcinoma might be involved in inducing the alopecia areata in
this patient.
